Cognitive Distortions Identification Worksheet

Become more aware of automatic thought patterns and practice replacing them with more balanced and realistic perspectives.

What Are Cognitive Distortions?

Cognitive distortions are automatic patterns of thinking that can negatively influence our emotions, behaviors, and decision-making. Everyone experiences distorted thinking from time to time, especially during periods of stress, anxiety, depression, or trauma.

The goal of this worksheet is not to judge your thoughts, but to become more aware of them and practice replacing them with more balanced and realistic perspectives.

Remember

Thoughts are not facts. Becoming aware of unhelpful thinking patterns allows you to respond with greater clarity, flexibility, and self-compassion. Changing long-standing thought patterns takes practice, but small, consistent steps can lead to meaningful improvements in emotional well-being.
